It provides clear information about how the process called “transition planning” works, helping the reader to navigate the formal system and encourageing a focus on the things that matter to them. It has been designed to encourage effective participation.
It also contains information that can help the professionals to understand the impact of deafblindness and, as a result, forge a positive working partnership with families and young people.
